Helping New Yorkers Access the Social Care They Need

New York State’s 1115 Medicaid Waiver expands how Medicaid can address health-related social needs—the everyday circumstances that can make it harder to stay healthy. These needs may include food insecurity, unstable housing, transportation barriers, utility needs, and personal safety.

In the Bronx, the United Way of NYC serves as the lead Social Care Network, coordinating screenings, navigation, and connections with community-based organizations. Utopia Resource Center helps make that process easier to understand. We listen, explain available options, and help people connect with appropriate community resources.
